مقدمة

الحمد لله رب العالمين والصلاة والسلام على رسول الله النبي الأمين وعلى آله وصحبه أجمعين

أخي المريض

نرحب بك في مستشفى جامعه العلوم والتكنولوجيا ونتمنى لك أفضل رعاية صحية شامله بأيدي خبراء وأجهزه حديثه ومتطورة وبكل إخلاص وأمانه وصدق، شعارنا في ذلك ملتزمون بالتميز.

المريض

هو الضيف الزائر الى المستشفى تعرض إلى وعكة صحية ويشكو من ألم أو خلل وظيفي يبتغي العلاج والشفاء بأقل تكلفة وأقصر مدة وأفضل وسيلة وبخطوات سهلة وميسرة يتوقعها في مستشفى جامعه العلوم والتكنولوجيا.

إرشادات للمريض

• أخي المريض: إذا كنت تشتكي من ألم حاد أو حالة طارئه أو أي حالة صحية تستدعي السرعة ولا تحتمل الانتظار عليك التوجه إلى قسم الطوارئ الموجود بالمستشفى وستجد السرعة في الإسعاف والفحص واتخاذ الإجراءات الطبية اللازمة من صور أشعه وفحوصات مختبريه وإذا استدعى وضعك الصحي البقاء في المستشفى يتم تحويلك إلى القسم المختص لإتمام الإجراءات الضرورية مثل العمليات الجراحية أو الرقود لعمل فحوصات تحتاج إلى بضعة أيام .
• أخي المريض: اذا كنت تشتكي من مشكله صحية بوجود أعراض مرضيه معينه تحتاج إلى تشخيص دقيق عليك التوجه إلى قسم العيادات الموجودة في المبنى (2) وسوف يتم ارشادك إلى عيادة الاختصاصي المناسب حسب حالتك الصحية . وهناك أكثر من (40) عيادة تغطي جميع التخصصات ليقوم بالإجراءات الطبية اللازمة.
• أخي المريض: إذا كنت تحتاج إلى فحص مخبري للحصول على نتائج مخبريه دقيقه وسريعه عليك الذهاب إلى قسم المختبر في الملحق (2) الطابق الخامس وسوف تجد خبراء في المحتبروباجهزه حديثه ومتطوره وفحوصات نادره لا تجدها إلا في مستشفى جامعه العلوم والتكنولوجيا.
• أخي المريض: إذا كنت بحاجه إلى صورة أشعه وصورة طبيقيه محوريه وصورة رنين مغناطيسي أو أي نوع من الصور الشعاعيه الأخرى توجه إلى قسم الأشعه الموجود في طابق البدروم وستجد خدمة متميزة بايدي خبراء متخصصين وبأجهزه حديثه.

الرقود في المستشفى وسياسة الدخول والخروج:

أولاً: سياسة الدخول:

• إحضار أمر الرقود للعملية الجراحية او الباطنية من الطبيب المختص في العيادات او من الطبيب الخارجي .
• التوجه إلى مكتب الدخول وحسابات المرضى في الدور الأرضي بالمبني الرئيسي او إلى حسابات الطوارئ في الفترة الليلية .
• الاستعلام عن التكلفة للعملية الجراحية والرقود والإجراءات الأخرى .
• اختيار الغرفة المناسبة في اقسام الرقود .

حقوق المريض :

• أن يُعامل بإحترام من جميع أفراد طاقم المستشفى دون أي تمييز وفي بيئة آمنة وخالية من العوائق.
• أن يتلقى الرعاية الصحية اللازمة من أجل الحصول على تشخيص سليم وعلاج لمرضه أو إصابته.
• أن يتم إعلامه باسم ووظيفة الطاقم الطبي الذي سيكون مسؤل عن رعايته.
• أن يتم إعلامه بتفاصيل وضعه الصحي بصورة كاملة على أن يقوم الطاقم الطبي بالتصريح بهذه المعلومات له شخصياً أو لمن ينوب عنه.
• أن يشارك في القرارات المتعلقة بالرعاية الصحية المقدمة له.
• أن يحصل علي جميع المعلومات التي يحتاج معرفتها لكي يتم الموافقة على إجراء أو علاج يتم اقتراحه عليه.
• أن يتم الحفاظ على خصوصياته أثناء اقامته في المستشفى وأن يتم الحفاظ على سرية جميع الأمور المتعلقة بالرعاية الصحية المقدمة له.
• أن يستلم فاتورة مفصلة وموضحة بجميع التكاليف.
• أن يطلع على سجلاته الطبية وأن يحصل على تقاريره الطبية عند الطلب مقابل رسوم مناسبة.
• أن يتسلم خطة الخروج من المستشفى تتضمن تفاصيل رعايته الطبية المستمرة.
• أن يتقدم بالشكوى بخصوص الخدمات والرعاية التي تقدم له إذا تم التقصير فيها وأن تستجيب إدارة المستشفي لشكواه.

واجبات المريض :

• أن يُظهر مراعاته للمحيطين به وأن يعامل المرضى والعاملين بالمستشفى بإحترام.
• الإلتزام بقواعد وأنظمة المستشفى.
• الإلتزام بسياسة عدم تناول القات.
• الإلتزام بسياسة عدم التدخين.
• الإلتزام بسياسة منع الدخول بالأسلحة النارية والبيضاء.
• الإلتزام بضوابط الزيارة لضمان الحقوق والراحة للمرضي.
• أن يحترم خصوصية المرضى الآخرين.
• أن يقدم معلومات كاملة وصحيحة عن شكواه وتاريخه المرضي وأي حساسية يعاني منها تجاه أي عقاقير طبية وجميع الأمور الاخرى المتعلقة بصحته.
• أن يقوم بالإبلاغ عن أي تغيير يطرأ على صحته أو أي شعور بعدم التحسن.
• أن يتبع الخطة العلاجية التي يصفها له طبيبه.
• أن يقبل تحمل المسؤلية عن رفضه للعلاج أو عدم إتباعه لخطة العلاج.
• أن يلتزم بالمواعيد المحددة أو يقوم بإلغائها مسبقاً.
• أن يتفهم أن حالات بعض المرضي قد تستدعي اهتماماً أكبر من حالته ولهذا السبب ستكون لهم الأولويه.
• أن يمتثل لقواعد السلامة العامة وسلامة المرضى بالمستشفى.
• أن يحرص على السداد الفوري للتكاليف المالية للرعاية الصحية التي يتلقاها.

معلومات هامه عن المستشفى :

• يتوفر في المستشفى جميع التخصصات الطبية .
• يتوفر في المستشفى اجنحه خاصه فندقيه متميزة.
• يتوفر في المستشفى سيارات اسعاف متكامله وحديثة.
• يتم عمل فحوصات مخبريه نادرة ولأول مرة في اليمن.
• نظام رعاية تمريضية وصحية حديثه ومتطوره.
• يتوفر صاله ألعاب خاصة للأطفال.
• يوجد في المستشفى أحدث الأجهزة العالمية والحديثة والمتطورة .
• يوجد نادي صحي للرجال والنساء.
• يوجد قسم العلاج الطبيعي.
• توجد عياده الورفارين.(مميعات الدم )
• يتوفر غرف خاصة للعزل والأمراض المعدية .
• تم رفد المستشفى بإستشارية جراحة الثدي والجراحة العامه.
• يوجد أقسام نادرة ولأول مرة في اليمن.

جغرافيه المستشفى

يضم المستشفى في ساحاته مباني متعددة موزعة كالتالي:-

• المبنى الرئيسي (1) ويتكون من سبعه طوابق مقسمة حسب الأتي :-

1.طابق البدروم: ويحتوي على الأقسام التالية (قسم الاسعاف والطوارئ رجال ونساء وأطفال كل قسم على حده - بنك الدم المركزي - قسم الأشعة - مكتب المناوب الإداري - صيدلية الطوارئ - قاعة د .طارق سنان الكبرى - كفتيريا الطوارئ - عمليات الطوارئ الكبرى .
2.الطابق الأرضي: ويحتوي على مركز النساء والولادة - مركز المسالك البوليه والتفتيت - مركز الجهاز الهضمي والمناظير - قسم المعالجة التنفسية - قسم غسيل الكلى والغسيل البريتوني - قسم الحالات اليومية - حسابات المرضى - مكتب الادخال - مكتب الحجوزات - الاستعلامات - كافتيريا واستراحة الزوار - مكاتب تنسيق الشركات - مختبر لسحب العينات - مكتب الأحوال المدنية.
3.الطابق الأول: يحتوي على قسم العناية المركزة – قسم العناية الحثيثة للقلب - قسم العمليات الرئيسية والتعقيم المركزي – مركز قسطرة القلب والشرايين.
4.الطابق الثاني: يحتوي على قسم الولادة - قسم حضانة الخدج والمواليد - قسم العناية المتوسطه رجال - قسم العناية المتوسطه نساء - قسم العناية العصبيه - قسم الأجنحه والرقود - السجلات الطبية والمحاسبة - إدارة التمريض .
5.الطابق الثالث: ويحتوي على قسم رقود الباطنية وجراحة نساء وأطفال - قسم الأجنحة والغرف الخاصه نساء – الصيدلية الداخلية - مكتب السجلات والمحاسبة .
6.الطابق الرابع: ويحتوي قسم الباطنية والجراحة – الأجنحة الخاصة رجال - استراحة الزوار - مكتب مدير خدمات المرضى.
7.الطابق الخامس: يحتوي على سكن الأطباء والمناوبين- حضانة الاطفال - النادي الصحي رجال ونساء - العلاج الطبيعي.

• مبني الملحق (2)

يقع مقابل البوابة الرئيسية للمستشفى ويضم الأقسام التالية :
- المطبخ المركزي - المغسلة
- - مجمع الإدارات - السجلات الطبية - المختبر المركزي
- العيادات التخصصية - الصيدلية الرئيسية
- مكتب التقارير الطبية - الاستقبال وخدمه العملاء

• مبني الملحق (3)

يضم المبني أقسام الصيانة والحركة والأجهزة الطبية

• مبني الملحق (4) ويضم الأقسام التالية:

المخازن المركزية - المسجد
مواقف السيارات - صالة الطعام

العناية الصحية أثناء الرقود في المستشفى:

يتم وضع المريض في القسم المختص حسب حالته الصحية وفي الغرفة المناسبة بحسب الدرجة التي يرغب بها المريض علما أنه يوجد عدة مستويات من الدرجات المختلفه وهي كما يلي:-
1. الأجنحة الملكية: غرفتين مزدوجتين إحداها غرفة للمريض والأخرى منفصله للزوار والمرافقين وتحتوي على أثاث متميز وخدمة فندقية متميزة.
2. الأجنحة: غرفة خاصة متسعة يكون المريض فيها لوحده وتحتوي على أثاث مميز وخدمة فندقية متميزة.
3. الدرجة الأولى والخاصة : وهي غرفة منفردة خاصة بالمريض وهي أصغر من الجناح من حيث المساحة وتحتوي أيضاً على أثاث وخدمات فندقية متميزة.
4. الدرجة لثالثه: ويرقد فيها ثلاثة مرضى وتحتوي على جميع الأثاث اللازم.
5. غرف العزل وفيها يرقد المريض فقط لمنع انتشار العدوى الى المرافقين والمرضى الآخرين و هي مجهزة حسب المعايير الطبية .
6. العنايات : تعتبر العنايات من الأقسام الهامة جدا ويتم استقبال الحالات الحرجة والخطرة حيث يتلقى المريض الرعاية والاهتمام تحت إشراف أطباء وممرضون على مدار 24 ساعة ويمنع بقاء المرافق بجنب المريض وهي موزعة كالتالي :

( العناية المركزة- العناية القلبية – العناية العصبية –العناية المتوسطة – العناية بالتسمم ألحملي ).

مرافق المريض

• يسمح بوجود مرافق واحد مع المريض شرط أن يكون رجل في قسم الرجال وامرأة في قسم النساء أو الأطفال.
• يمنع وجود المرافق الثاني حرصا على مصلحة مريضك ولان ذلك يزيد عليك التكاليف .
• لا يسمح بوجود مرافق في اقسام العناية المركزة او الخداج او المواليد لوجود ممرضين وممرضات بجوار كل مريض ولوجود اجهزه مراقبه دقيقه.
• لا يسمح بوجود مرافقين في قسم الولادة ويمنع دخول الرجال.
• يتم توفير وجبات طعام للمرافق وكرسي مريح للنوم وبطانية ومخدة إضافة إلى خدمة النت في الغرف الخاصة.
• يمكن للمريض أو المرافقين طلب وجبات إضافيه من قسم التغذيه عن طريق ممرض القسم .
• لا يسمح ببقاء المرافقين في غرف العزل إطلاقا حفاظاً على صحتهم ومنعاً لانتشار العدوى إليهم.

خدمات غرف المرضى

• يتوفر في جميع غرف المرضى حمام، تلفزيون، جرس، ثلاجة، كراسي زوار ، مصحف، سجادة.
• يتم توفير خدمات إضافية عند الطلب للمريض (كرسي متحرك، كرسي حمام، مبوله، عكازات، بطانية إضافيه، وجبة طعام إضافية، ماء ).

حاجات المريض الصحيه أثناء الرقود :

• يقوم على رعايه المرضى الرجال ممرضون ذكور والمرضى من النساء والأطفال ممرضات إناث وذلك في جميع الأقسام.
• يتوفر جرس عند رأس كل مريض يضغط عليه عند الحاجه إليه فيحضر الممرض أو الممرضه بأسرع وقت ويقوم بتلبية الطلب .
• يقوم كادر التمريض بتغيير ملايات الأسره يومياً وعند الحاجة إلى ذلك.
• يقوم كادر التمريض بقياس الضغط والنبض والحرارة والتنفس للمريض دورياً وحسب أوامر الطبيب للاطمئنان المستمر ومتابعة العنايه والعلاج.
• يقوم الأطباء والممرضون والاختصاصيون بعمل جولات دورية على جميع المرضى والمتابعة والاطمئنان على المرضى وعمل الفحص اللازم حسب الخطة العلاجية والحاجة المرضية.
• يقوم التمريض بالتنسيق مع الطبيب المناوب باستمرار طوال 24 ساعة لمتابعة جميع المتغيرات الإيجابية والسلبية واتخاذ الإجراء اللازم.
• يقدم الكادر التمريضي الأدوية والعلاجات اللازمة في وقتها وبالطرق العلمية الصحيحة.
• يحفظ الملف الطبي للمريض عند الممرض المسؤول وليس عند المريض للحفاظ على التقارير والنماذج والفحوصات الطبية وللحفاظ على سرية المعلومات الخاصة بالمريض.
• يسمح للمريض بطلب الطبيب الاختصاصي الذي يريده ويسمح له او لولي الأمر تغيير الطبيب الاختصاصي ضمن نموذج خاص وبحسب اللائحة الداخلية للمستشفى .
• اذا أراد المريض او ولي امره طلب استشاره اختصاصي أخر غير طبيبه المعالج فله ذلك ضمن نموذج خاص مع إضافة رسوم إضافية للخدمة.

نصائح ضرورية للمريض:

يجب على المريض الالتزام بجميع التعليمات الطبية من الطبيب المعالج أو الممرض والتعاون التام مع الكادر الطبي والتمريضي في أخذ الأدوية وعمل الإجراءات اللازمة وتناول الطعام وذلك من اجل الشفاء العاجل إن شاء الله والتقليل من المضاعفات.
اعلم أخي المريض أن الكادر الطبي والتمريضي خبراء في مجالاتهم وبكل إجراء يقومون به لهم هدف هام وإيجابي يساهم ويساعد في سرعة شفائك بإذن الله تعالى فكن مطيعاً متعاوناً معهم وذلك لما فيه مصلحتك.

تعليمات هامة:

• يمنع منعاً باتاً التدخين وتناول القات داخل المستشفى.
• يمنع تناول أي علاج داخل المستشفى إلا بإذن من الطبيب المعالج أو الممرض المسؤول حتى لا تتداخل معطيات العلاج وتسبب لك ضرر صحي.
• الالتزام بتعليمات السلامة الموجودة في المستشفى.
• تعرف على مخارج الطوارئ بالمستشفى.
• لا تستخدم تمديدات كهربائية من نفسك داخل غرفتك.
• لا تستخدم أي مواقد للنار من تلقاء نفسك في غرفتك.
• في حالة السعال أو العطاس استخدم مناديل ورقية.
• بعض الغرف قد يوجد على أبوابها لوحات أو كروت ملونة اسأل الممرض أو الممرضة أولاً عن مدى مأمونية دخول تلك الغرف.
• لو وجدت أي سوائل مسكوبة في أرضية غرفتك اطلب عامل النظافة فوراً.
• تجنب لمس حاويات المخلفات البيولوجية صفراء اللون.
• تجنب لمس الحاويات المخصصة لإلقاء الإبر والمحاقن.
• لا تتردد في طرح أي أسئلة على المختصين أثناء فترة إقامتك.
• إذا كان لديك حساسية من بعض الأدوية أو التخدير يجب أن تخبر الطبيب والممرض.
• احتفظ دائماً بنسخة من الأشعات والفحوصات.
• اسأل عن نتائج التحاليل والأشعات التي أجريت لك.
• لا تخجل من تنبيه طواقم المستشفى عند تقديم الخدمة إذا لم يرتدي القفاز.
• عند استلام دواء الخروج يجب أن تتأكد انه الدواء المطلوب فعلاً.
• إذا تقرر لك إجراء عملية جراحية فاسأل عن تفاصيل الإجراءات الجراحية التي سيتخذها لك الطبيب.
• يجب أن تسأل طبيبك عند خروجك عن الخطة العلاجية وعن الممنوعات وعن بعض الأنشطة مثل الاستحمام والحركة وقيادة السيارة وغيرها.

طعام المريض

حفاظاً على صحة المريض من نقل العدوى والجراثيم يمنع منعاً باتاً ادخال أي طعام أو شراب من خارج المستشفى والالتزام بالوجبات الغذائية المخصصة لك من قبل الطبيب المعالج حسب حالتك الصحية لأن هناك أخصائيين تغذية يقومون بإعداد الوجبات للمرضى ضمن قواعد علمية وحسب أوامر الطبيب الاختصاصي.

زياره المريض:

هناك مواعيد محدده لزيارة المرضى ولفترات محدودة وذلك من أجل توفير الراحة والهدوء لك ولبقية المرضى وحفاظاً على سلامة الجميع ومنعاً للإزعاج وإيذاء الآخرين:
• يمنع اصطحاب الأطفال حفاظاً على صحتهم من نقل العدوى.
• يجب توفير الراحة والهدوء حفاظاً على سلامة الجميع ومنعاً للإزعاج.
• زيارة العنايات المركزة مرة واحدة في اليوم ولقريب المريض من الدرجة الأولى فقط .
• تنتهي الزيارة في تمام الساعة التاسعة مساء .

أمانات المرضى:

- أخي المريض حفاظاً على ممتلكاتك الخاصة فقد تم توفير مكتب خاص بهذه الأمانات لوضعها فيها وذلك مقابل استلام سند بذلك ضماناً لها من الضياع في البوابة الرئيسية .

صلاة المريض:

أخي المريض اعلم أن المرض ابتلاء من الله سبحانه وتعالي والصحة نعمة عظيمة، والصلاة عبادة وفريضة لا تسقط عن المريض إطلاقاً وإذا كنت لا تستطيع الوضوء أو إن الماء يضرك لوضعك الصحي فعليك بالتيمم وإذا كنت لا تستطيع الصلاة واقفاً فصل وأنت جالس وإذا كنت لا تستطيع فصل وأنت راقد في السرير بالإشارة، وإذا احتجت إلى مساعدة في الوضوء أو التيمم فاطلب ذلك من الممرض الموجود في القسم .

أقتراحات

أخي المريض عملاً بمبدأ رحم الله من أهدى إلي عيوبي إذا كان لديك أية مقترحات أو شكاوي أو ملاحظات سواء كانت إيجابيه أو سلبية اكتبها في الاستمارة المخصصة لذلك وسلمها إلى المسؤول أو إلى خدمة العملاء في الفترة الصباحية .كما يمكنك وضعها في صندوق المقترحات أو إلى مراقب التمريض على الفترة المسائية أو إلى مسؤول القسم الذي ترقد فيه وسوف يتم متابعتها بجديه.

سياسة الخروج :

اخي المريض عند ابلاغك بالخروج عليك اتباع الخطوات الاتية :
• انتظر الطبيب المقيم في القسم حتى يتم الانتهاء من المجارحة للمرضى الاخرين وكتابة وصفة الخروج الخاصة فيك .
• استلم دوائك من التمريض في القسم قبل الخروج .
• انتظر حتى يتم استكمال اجراءات الخروج من قبل محاسب الطابق والكاتب واصدار فاتورتك المالية .
• التوجه الى حسابات المرضى لسداد ماعليك واخذ تصريح الخروج .

The Patient

the patient is a` visiting guest to the hospital who has a health disorder and complains of pain or dysfunction seeking treatment and healing at the lowest cost, shortest time , best way and easy and simple steps expected in the hospital of the University of Science and Technology.

Instructions for the patient:

Dear patient

• If you complain of severe pain, emergency or any medical condition that calls for urgency and cannot stand to wait, go to the emergency department located in the hospital and you will find treated very fast where you will undergo examinations and the necessary medical procedures such as the radiological images and laboratory tests.

In the event that your condition require admission to stay in the hospital for further management , You will be referred to the appropriate department to complete the necessary procedures, such as surgeries or tests for a few days.

• If you complain of a health problem with certain symptoms, you need to have a thorough diagnosis. You should go to the OPD building . You will be guided to the appropriate specialist clinic according to your health condition. There are more than (40) clinics covering all disciplines to perform the necessary medical procedures.

• If you need a laboratory test to get accurate and fast laboratory results, you should go to the lab Department in the Building 2 on the fifth floor and you will find Lab experts using the most advanced laboratory machines and devices which can analyze rare tests found only in UST Hospital .

• If you need a radiological imaging such as MRI , CT Scan , US, X-ray, etc. , you need to visit The basement floor in the main building of the hospital . the department is staffed with very qualified doctors and technicians and it is equipped with the most up to date machines all over Yemen.

Admission in the Hospital
